What to use for lightweight bug/issue tracking?

What do people use for bug tracking? Ideally we'd want something super lightweight with as little of a learning curve as possible.

We use Google Docs occasionally, but would like something a little (just a little, mind you) less ad-hoc.

Some folks on this list have teams of developers, some teams include journalists, some include designers. So, given the range of skills that could be useful, what would you include in an ideal mix? And what's a good size for a news apps/interactive team?

Semi-comprehensive list of news development teams

This isn't really a question, but I thought this would be a good place to put this. We've had several threads on the NICAR-L listserv recently about who has created news development teams.

I'll take a shot at categorizing these, but feel free to offer critiques and additions, and a revised list will probably be in order.

And though I can't really think of a reason, if you'd prefer we don't list your team here, please let me know. I'd include names, but those would probably change a lot. Can this here site have a wiki component?
